Health Leads

Health Leads partners with communities and health systems to address systemic causes of inequity and disease by removing barriers that keep people from identifying, accessing, and choosing the resources everyone needs to be healthy. With support from RWJF, Health Leads will support caregivers, help build community coalitions of community members, and strengthen data and measurement practices for those coalitions in its effort to become a learning and innovation hub that helps health systems and also community organizations address the social determinants of health and root causes of racial inequity that impact health.
