This case study highlights the Communities That Care Coalition in Franklin County, Massachusetts, an evidence-based, prevention-science program that guides communities through a five-phase change process to promote healthy youth development, improve youth outcomes, and reduce problem behaviors.
While reducing risky youth behaviors and promoting healthy habits has been the focus, the coalition participates in all types of prevention strategies, building connections among youth, families, schools, and communities to improve youth health and health equity.
Aligning Systems for Health: Health Care + Public Health + Social Services, supported by the Robert Wood Johnson Foundation and managed by the Georgia Health Policy Center, is focused on learning from stakeholders across the nation about effective ways to align these three sectors to better meet people’s goals and needs.
