100 Million Healthier Lives is a collaboration of change agents across sectors who are pursuing an unprecedented result. The effort aims to transform the way the world thinks and acts in order to improve health and well-being and create 100 million healthier lives by 2020.

Over the past four years, Medicaid and public health teams from 40 states, local jurisdictions, and territories across the nation have forged new partnerships under the 6|18 Initiative of the Centers for Disease Control and Prevention. The Initiative helped these state teams make progress in aligning previously siloed cross- sector activities to achieve sustainable improvements in care for people with high-burden, high-cost health conditions.

To improve the health outcomes and daily lives of individuals in the United States with the most complex care needs, seven leading health care foundations—Arnold Ventures, The Commonwealth Fund, The John A. Hartford Foundation, Milbank Memorial Fund, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation—are working together to accelerate health system transformation.

The Center for Health Care Strategies is evaluating a multisite demonstration project in settings for pediatric primary care; supporting a partner organization that can offer technical assistance to the grantee and the pediatric sites in addressing the social determinants of child and family health and in elevating family voice; and sharing insights widely on how to catalyze wide-scale system transformation.

Advancing Health Equity (AHE) is a national initiative that supports teams of Medicaid providers, Medicaid managed care organizations, and state Medicaid staff to advance health equity through implementation of new models that emphasize whole-person care. AHE is convening teams of leaders in seven states to design integrated payment and health care delivery reforms to reduce health disparities.

Advancing Integrated Models (AIM) is a national multi-site demonstration assisting health systems and community providers in integrating innovative, “next- generation” person-centered approaches for individuals with complex health and social needs. AIM includes eight competitively selected pilot sites representing innovators in care for both pediatric and adult populations.

The Advancing Resilience and Community Health (ARCH) Initiative of the Nonprofit Finance Fund provides technical assistance, resources, and knowledge-sharing across multiple stakeholders to help the burgeoning relationships between health care institutions (payers and hospital systems) and community-based organizations (CBOs) get off the ground successfully and at scale. ARCH is helping ready networks of CBOs—nonprofits that have already come together to address multiple needs—to collectively contract with often-larger health care institutions, with an emphasis on contracts involving Medicaid.

The Center for Health Care Strategies is partnering with the National Association of Medicaid Directors and ZERO TO THREE to create a learning collaborative with seven to nine state teams in order to improve outcomes for young children. The collaborative lasts two years and includes teams committed to achieving stronger alignment at the state and local levels between Medicaid stakeholders and early childhood systems focused on children from the point of their birth through the first three years of life.

Aligning Systems for Health focuses on identifying, testing, and sharing what works to align health care, public health, and social services in order to better address the goals and needs of the people and communities they serve. Aligning Systems for Health will continue to translate insights for communities, researchers, and decision-makers to support on-the-ground efforts aimed at greater coordination among systems.

Ascend at the Aspen Institute is a hub for breakthrough ideas and collaborations that move children and their parents toward educational success and economic security. Ascend takes a two-generation approach to its work—focusing on children and their parents together—and it brings a gender and racial equity lens to our analysis.
