This case study describes how the Denver Regional Council of Government’s (DRCOG’s) Area Agency on Aging (AAA) expanded its existing network to become the first AAA in the nation to lead an Accountable Health Community.
The DRCOG’s AAA has a nearly 50-year history of aligning social services for the area’s growing number of seniors and adults with disabilities. It serves as a community hub providing a comprehensive, coordinated system of community-based services and supports, both through direct provision of services and through contracts with partner community-based organizations.
Aligning Systems for Health: Health Care + Public Health + Social Services, supported by the Robert Wood Johnson Foundation and managed by the Georgia Health Policy Center, is focused on learning from stakeholders across the nation about effective ways to align these three sectors to better meet people’s goals and needs.
