This case study features People’s Community Clinic, a Federally Qualified Health Center based in Texas that uses a patient-empowerment framework that enables patients and families to take control, not just of their health, but also of their lives to optimize well-being.
Through the Community-Centered Health Homes initiative, with support from the Episcopal Health Foundation, People’s engages in community organizing to build neighborhood champions that can help address upstream, systemic determinants of health in partnership with the clinic.
Aligning Systems for Health: Health Care + Public Health + Social Services, supported by the Robert Wood Johnson Foundation and managed by the Georgia Health Policy Center, is focused on learning from stakeholders across the nation about effective ways to align these three sectors to better meet people’s goals and needs.
